The South West Aboriginal Medical Service (SWAMS) is a Noongar Community Controlled Health Organisation, founded on the principles of self determination, empowerment and freedom of choice. Their mission is to provide high quality, culturally appropriate and holistic healthcare which builds strong and healthy Aboriginal Communities.

Kalil Mordidjabiny Program

The Kalil Mordidjabiny (Home becoming strong) / Transition Care Program (TCP) is a time-limited, goal-orientated program designed to improve or maintain independence and confidence after a stay in hospital.

Bindjareb Aged Care Services (Mandurah)

The first Aboriginal Aged Care Service in Perth was launched in Mandurah on Wednesday, 1 February 2023. The service will be the hub of home and community care activities, services and supports for First Nations Elders in the Bindjareb region.

Health Service

They are an Aboriginal Community Controlled Health Organisation, founded on the principles of self-determination, empowerment and freedom of choice. They are committed to advancing the health and wellbeing of our mob, as a whole from the very beginning. They want to assist in bridging the gap between cultural and mainstream services.
